Diabetic retinopathy is one of the leading causes of vision loss among adults with diabetes, yet it often progresses silently in its earliest stages. Understanding how it develops, and the importance of regular screening, can make the difference between early treatment and irreversible vision loss.

What Is Diabetic Retinopathy
Diabetic retinopathy occurs when consistently high blood sugar levels damage the small blood vessels in the retina. Over time, these vessels can weaken, leak, or become blocked, affecting the retina’s ability to function properly.

The Stages of Diabetic Retinopathy
Mild Non-Proliferative Retinopathy: small areas of swelling (microaneurysms) appear in the retina’s blood vessels. Moderate Non-Proliferative Retinopathy: blood vessels supplying the retina become increasingly blocked. Severe Non-Proliferative Retinopathy: a larger number of blood vessels are blocked, cutting off blood supply to significant areas of the retina and signaling the retina to grow new vessels. Proliferative Diabetic Retinopathy: the most advanced stage, where abnormal new blood vessels grow on the retina’s surface. These vessels are fragile and prone to leaking, which can lead to significant vision loss if untreated.

Symptoms to Watch For
In its early stages, diabetic retinopathy often causes no noticeable symptoms at all, which is exactly why screening matters. As it progresses, symptoms can include blurred or fluctuating vision, floaters, dark or empty areas in your vision, and in advanced cases, sudden vision loss.

Who Is at Higher Risk
Risk increases with the duration of diabetes, poorly controlled blood sugar levels, high blood pressure, high cholesterol, and pregnancy in women with diabetes.

Treatment Options
Treatment depends on the stage and severity, and may include anti-VEGF injections to reduce abnormal blood vessel growth and swelling, laser photocoagulation to seal or shrink leaking vessels, and vitrectomy surgery for advanced cases involving bleeding or scar tissue in the eye.

Why Regular Screening Matters
Because early diabetic retinopathy rarely causes symptoms, anyone living with diabetes should have a comprehensive dilated eye exam at least once a year, even with no vision complaints. Early detection is what keeps treatment options simple and effective.
